A deterioration score built from EHR data tests well, and a review of its strongest inputs shows blood culture orders and lactate requests near the top. Those entries record that a clinician was already worried, so part of what the model learned is to echo suspicion the team already has, and it adds least for patients nobody has thought to test yet. When reviewing any EHR-based tool, ask which inputs record clinician actions such as orders, and how well it performs for patients before any of those actions have happened.
