Operating threshold

The cut-off that decides how readily a predictive tool flags a case. Lower it and you catch more, at the cost of more false alarms; raise it and you cut false alarms, at the cost of missed cases. Where it sits is a clinical decision, not a fixed property of the tool.

In the clinic

A sepsis screening tool arrives with a default threshold the developer chose on its own data, and nobody at the hospital is named as owner of that setting. The emergency department wants it lower so fewer septic patients slip past triage, while the orthopedic ward, where fever and a raised heart rate are common after surgery, wants it higher because alerts already pile up there. Agree on a threshold for each setting with the clinical leads, write down the expected alerts and misses at each, and name who reviews it when case mix changes.
